Note: Each day there will be health check at the reception before accepting a child. Every day you may have to complete above form. We will not accept the child without a completed form. You may download this form and be ready with all the answers when checking in. Body temperature will be taken at that time. If you prefer you can bring your own thermometer. Only one person is allowed inside the drop off area at a time. Detailed instructions are posted at the daycare. Please read and follow them. Please DO NOT bring the child to the daycare if answers to any of the questions from 7 to 13 is a 'Yes' or the body temperature of either of you or the child is 100.4° Fahrenheit or more.
GUIDELINES TO PARENTS ON REOPENING
We are excited to re-open. Since the threat from COVID-19 pandemic is still not over, we may have to exercise extreme caution while opening up. To protect children, staff and parents and following directives from different government agencies, we have developed following guidelines for parents. These guidelines will be reviewed from time to time based on our experience and further directives from the government. Your co-operation is extremely important for ensuring safe environment for yourself, children and the staff.
Procedure to Drop Off:

Follow the same procedure as laid down for ‘Drop Off’ above in items I to VIII

As parent/guardian I understand and agree to follow above guidelines strictly and completely. By signing this document, I also agree to the following:
Procedure to Drop Off:
- Only one parent is allowed inside the drop off area at a time. When someone is inside, a red light located on the side of the entrance steps will be turned on. You CANNOT ENTER inside the daycare if that light is on. In that case you reach the front door through the handicapped ramp and,
- Pick up a number from the dispenser
- See through the glass door inside what number is being served
- Wait for your turn in your car
- Enter the daycare upon your turn when the red light is NOT on.
- Sanitize your and the child’s hands upon entrance. A sanitizer dispenser is available inside right at the entrance door.
- You must wear a face mask all the time when in the drop off area. If you do not have a face mask you cannot enter.
- Children above two years of age must also wear a mask.
- A touchless thermometer is installed at the reception desk. Your and the child’s body temperature will be taken. If the temperature is 100.4 or above, we will not accept the child. Parents can bring their own thermometer. Body temperature of both the parent and the child will be taken in front of the staff.
- Submit the ‘Health Check Questionnaire’ at the reception everyday. Based on your answers entry will be given or may be denied.

Terms and Conditions:
As parent/guardian I understand and agree to follow above guidelines strictly and completely. By signing this document, I also agree to the following:
- To minimize drop off time, I will have a completed health check questionnaire before taking body temperature in front of the staff member
- Blank health check forms will be provided weekly in advance by the center. If I do not have a form at the time of drop off a fee of $10.00 will be charged for providing another form. I will take the form and complete it outside the building and take a new number to come in again as per the process laid down above.
- I will not come to the center if I or the child have a temperature of 100.4 degrees Fahrenheit or above.
- I will not enter the drop off area without a face mask. Children over 2 years of age must also wear a face mask.
- Parents are not allowed beyond the drop off area in the daycare center at any time. The open door policy is suspended at this time.
- If the child gets sick or unwell for whatever reason while in the center, I undertake to pick him/her up within 30 minutes. I understand the center may call police, the health department or any other appropriate agency after 30 minutes wait period.
- Tuition fee becomes due on every Monday for the whole week in advance. A late fee of $20.00 will be charged after that. If the fee is not paid by Tuesday, the child will not be accepted on Wednesday.
- In order to ensure safety and efficient execution of elaborate procedures I understand that Police may be called in case of inappropriate behavior.
- Waiver: I am dropping off the child at the center with the awareness that the COVID-19 virous is highly contagious and may be fatal. I understand that in a daycare setting, chances of getting infected are much higher than staying home following social distancing and other safety measures. I agree to hold the daycare center or any of its staff members harmless in case I or my child get infected.
